New Patients
All new patients need to complete the form GMS1 and a New Patient Health Questionnaire. These forms can be collected from the surgery or can be found on the above “Registering Online” tab.
You will also need to show 2 forms of identification. Photo ID and utility bill with your address on it. Until you have completed the forms and shown ID you are not fully registered with the Practice.
